Preparing for Your Visit
While preparing for your visit, keep the following in mind.
What to wear:
Wear loose-fitting clothing so doctors can perform exams easily.
Gowns will be provided if needed.
What to bring:
Your list of questions and concerns
List of medications you take, including over-the-counter medications and vitamins
List of prior surgeries and procedures
Insurance cards and photo ID
Pertinent medical records
What to expect:
Initial appointments last about an hour but can vary based on tests required.
If you’re having an ultrasound, you can eat and drink as normal.
If you require surgery, we can usually schedule you within a week or two of your appointment.
Parking Information
Central/Swedish Office: the Synergy building is across the street from Swedish hospital and has an attached parking garage. Please bring your ticket to the office and we will validate for you. There is also street parking on Pearl St. and Pennsylvania St. The main entrance to the office is on Pennsylvania St.
North/Aurora Office: there is ample parking around the office.
South/Skyridge Office: there is not a lot of up close parking but valet is available.
West/St. Anthony Office: there is parking around the office as well as valet parking.

Your medical records are entirely confidential. CCVSA will not release any information without your written consent.
The Health Insurance Portability and Accountability Act (HIPAA) requires all health care providers to inform their patients of their confidentiality policies.
Patient Portal Access
We use HealthCare Support Portal to facilitate better communication with your physician’s office by providing convenient 24/7 access from the comfort and privacy of your home or office.
Payment & Insurance
CCVSA accepts a variety of insurance programs, including those listed below. Payment is due at the time of service. If you do not have insurance, you can arrange a payment schedule with our billing department. We accept VISA and Mastercard.
